What is a "patient-centered medical home"?

A patient-centered medical home is a model of care that strengthens the physician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship.

Each patient has an ongoing relationship with a personal physician who leads a team at a single location that takes collective responsibility for patient care, providing for the patient's health care needs and arranging for appropriate care with other qualified clinicians. NC Family Physician of the Year

Dr. Charles Rhodes of Mt. Pleasant, NC, was named the 2009 NC Family Physician of the Year. Dr. Rhodes was recognized for embracing the ideals of family medicine which have included his lifelong commitment to comprehensive and coordinated care of his patients, teaching residents and medical students, dedication to mission work overseas, and serving his community.

Cabarrus Family Medicine Supports Future Leaders

For the fifth consecutive year, one student at each high school in Cabarrus County was awarded the Cabarrus Family Medicine College Scholarship.  This year we have extended the scholarship to North Mecklenburg, Stanly and Rowan counties. Cabarrus Family Medicine began this scholarship program to honor some of its county’s future leaders. Read more.
