AUTHORIZATIONS & NOTIFICATIONS If you are a new patient, you will need to fill out an Authorizations & Notifications Form. Here, you will list those who you authorize to have access to your information (family members, guardians, etc.). You may bring the completed form to your first appointment or turn it in ahead of time, by dropping it off or mailing it to your Cabarrus Family Medicine clinic. |
